The Adirondack Health patient portal facilitates better communication with your physician's office by providing secure and convenient access from the comfort and privacy of your own home or office.
Our health centers offer an independent patient portal using their electronic medical record software, e ClinicalWorks (eCW). The information in this portal pertains to care received at the health center. To request access to or assistance with the eCW patient portal, please contact the health center’s office during normal business hours.
A) If you are a HEALTH CENTER patient, click here to access your patient portal.

The hospital participates in the Health Information Exchange of New York State (Hixny). Part of this exchange includes a patient portal for patients to access their medical records. Currently the content includes Adirondack Medical Center inpatient and outpatient hospital records including but not limited to: medication lists, allergy information, inpatient and outpatient laboratory results, and discharge instructions.
In addition, if you have been treated at another Hixny participating facility, records from those facilities will also be available to you based on the nature of information they have agreed to include in this portal.
If you were not enrolled in the Hixny patient portal during your registration process and would like access, please complete the proper form below and mail to the Health Information Management Department, along with a copy of your identification. Completed forms can be mailed to:
